There is a one time enrollment fee of $75 per individual and $150 per family. Minimum membership length is 3 months.

What does the membership plan include?
Your membership includes all primary care services provided in our office. Additionally, we offer a wide range of care and care coordination through phone, email, and text, including virtual visits when appropriate.
